Keg worth Air Disaster

A 4 month old British Midland Boeing 737 crashed on a motorway in Nottinghamshire UK after the flight crew wrongly diagnosed a fire in the right hand engine. Captain Hunt who had over 15 years experience make a quick decision with little discussion or diagnosis with the co pilot, to close down the right hand engine. Passengers and cabin crew could see that the left engine was on fire but failed to tell the Flight Deck.
Rarely is a quick decision a good decision. Instead decision making models should be employed to consider a wide variety of options. Authority gradients can work against inclusive decision making
