Herald of Free Enterprise - Capsized Passenger Ferry

On 6th March 1987, the roll on roll off ferry sailed from Zeebrugge with her bow doors open. The ferry capsized and 193 died in the icy waters. The assistant boatswain fell as sleep and did not close the bow doors. The First Officer broke company procedures and was not present on deck to confirm the doors were closed.
The Captain assumed that all doors were closed. The Captain and the Crew on the bridge demonstrated poor situational awareness of the ships perilous configuration.
